- To help expedite your first appointment, please arrive a few minutes early to complete any necessary registration forms (if not already completed) and to verify your insurance, if applicable. This will ensure we have all the information needed to provide care for you or your child. For your convenience, you may also download and print the forms from our website, fill them out in advance, and bring them with you to your first appointment.
- Please note that the Acknowledgment Form must be reviewed and signed prior to establishing care.
